Before you apply

A Squared Medical Corporation, PC accepts applications from adults age 18 or older who live in any U.S. state. You may apply from your state, but that does not mean clinical care is available there. If we consider an offer of care, we will first determine whether the particular service can lawfully be provided where you are. Applying does not guarantee acceptance or reserve an appointment or blood draw.

The application asks for your email, name, date of birth, address, goals, symptoms, conditions, medications, allergies and family health history, and may ask about mental, reproductive or sexual health. We save answers as you go so you can return. Authorized practice personnel use them to assess your application and contact you about next steps. Service providers support account access, storage and necessary email. Please give only the requested information about relatives, without their names or other unnecessary identifiers.

Your application is for consideration only. It does not create a patient–clinician relationship, provide medical advice, enroll you in care, start a membership, authorize payment or treatment, or book an appointment or blood draw. The application is not monitored for emergencies. For an emergency, call 911 or your local emergency number.

The privacy-notice acknowledgment, terms agreement and any required health-data consents are separate actions. None is consent to treatment, research or marketing.
