What these terms cover
A Squared Medical Corporation, PC operates The Systems website and application. These terms govern browsing and applying for consideration. Our Website and Applicant Privacy Notice, Consumer Health Data Privacy Notice and separate Notice of Privacy Practices describe information handling and rights.
Who may apply
You may apply if you are at least 18 and live in a U.S. state. Give accurate information about yourself and keep your sign-in email under your control. You may be asked to verify information relevant to the application. Applying from any U.S. state does not mean a clinician can provide care in that state. We assess service availability under applicable licensing and other rules before offering care.
Do not include relatives' names or other identifying details unless a field expressly requests them. Do not use another person's account or access information without authority. We may restrict unauthorized or harmful use, while preserving your legal rights to your information.
Application and decision
You can save a draft and submit it for consideration. A submitted application may be reviewed by authorized practice personnel. We may contact you for clarification or next steps. We do not promise an answer or decision within a particular time, acceptance, a place in care, or availability of a specific clinician or service. You may withdraw an application; withdrawal ends consideration but does not automatically delete records that must lawfully be kept. The privacy notices explain requests and retention.
An application does not create a patient–clinician relationship. It is not a purchase, membership agreement, treatment consent, prescription request, lab order, appointment booking or blood-draw booking. Any later offer of care requires separate eligibility, terms, notices and consents. You do not agree to those future documents by submitting this application.
Medical information and emergencies
Website and application information is general and is not diagnosis, treatment or personal medical advice. The application is not monitored for emergencies. Do not send urgent symptoms through it or an ordinary email address. Contact your clinician for medical advice; call 911 or your local emergency number for an emergency.
Messages and account security
We may send sign-in codes and application-related service messages to the address you provide. They do not enroll you in marketing. Protect your email account, sign out on shared devices and tell support@thesystems.com if you suspect unauthorized access. Do not send health history to that ordinary email address.
Changes and contact
We may update these terms prospectively, show the new effective date and request a new agreement where required. We do not retroactively enroll you in care, authorize a charge or expand permission to use health information through a terms update. If you already submitted, the version you agreed to remains part of that submission record.
