Website and Applicant Privacy Notice

Who operates this service

A Squared Medical Corporation, PC operates The Systems website and application. In this notice, “we” and “our” mean that corporation. This notice explains information we handle when you browse thesystems.com, contact us, create an application account or apply for consideration. Our Notice of Privacy Practices separately explains rights and duties for protected health information. An application is for consideration; it is not enrollment in care.

Adults age 18 or older who live in any U.S. state may apply. Applying from a state does not mean that we can offer clinical services there. We will determine whether a particular service is available to you under applicable licensing and other requirements before offering care.

Information and sources

Website and device information. When your browser requests a page or website file, hosting and delivery services receive information such as IP address, browser details, requested resource and request time. The public site stores your film sound choice in browser session storage. The application uses technical information needed for sign-in, session security and service operation.

Information from you. We receive your email and sign-in activity; name, date of birth, gender and home and any voluntarily supplied secondary address; application answers about your goals, symptoms, conditions, medications, allergies, health history and family history; and your communications, requests and choices. The form may ask about reproductive, sexual or mental health. Family history can concern relatives. Give only the information requested, and avoid relatives' names or other unnecessary identifiers.

Application and review records. We create draft and submission status, timestamps, notice and consent records, staff review notes, decisions, privacy-request records and records needed to protect and administer the application. We may receive information you direct to us or that is needed to investigate a security or privacy issue. This notice does not state that outside laboratories, EHRs or other clinicians feed the application.

Purposes

We use this information to deliver the site, authenticate and protect your account, save and review an application, determine whether to contact you about next steps, answer your requests, maintain required records and investigate security or service problems. Age and address help assess application eligibility and where service might lawfully be available. Health and family-history answers help authorized practice staff assess fit; they are not used to establish a diagnosis through this application.

We do not use application health answers for targeted advertising, sell them or use them to train a general-purpose AI model. Applying does not subscribe you to marketing. Sign-in and application-related messages are service communications; promotional messages require their own lawful permission. We do not use advertising pixels or session replay on the application.

Recipients and sharing

Authorized practice personnel may see information needed for assigned application, support, privacy or security duties. Hosting, database, authentication and email providers process information needed to operate these functions under applicable agreements. Their access is limited to the service purpose; this notice does not establish that any particular vendor agreement has been executed. We do not share applicant health information with advertising networks or affiliates for their own marketing.

We may disclose information when required by law, or when a specific legal permission applies to security, a legal claim or a lawful request. We apply any stricter rule governing the particular record. We do not treat an application checkbox as authorization for unrelated disclosure, treatment, research or marketing. If additional, optional sharing is proposed, we will explain it and request any separate permission required before it occurs.

Storage and retention

The sound preference lasts for the browser session. Technical, account and correspondence records are kept for the time needed to provide and protect the service, handle a request, meet an applicable recordkeeping duty or preserve a documented legal hold. The record type and purpose determine the period.

Our application baseline is 90 days without applicant activity for health answers in a never-submitted draft, subject to an applicable right to earlier deletion or a documented legal exception. Submitted, declined and withdrawn applications have a seven-year baseline measured from the last substantive case action, subject to applicable law, a valid deletion right and documented holds. Routine reads, sign-ins and backups do not restart that clock. A record incorporated into later care follows the applicable clinical record rules. Backup copies have restricted access and follow the applicable recovery and deletion process; a backup is not an excuse to restore information that should remain deleted or restricted.

These baselines are handling rules, not a claim that every application is a medical record or that law requires seven years for every applicant. We evaluate the record and applicable law when a deletion request is made. We do not keep an applicant record solely because a default period has not elapsed when an enforceable right requires a different result.

Your choices and rights

You may choose not to apply and may leave optional questions unanswered where the form permits. You can request access, correction, withdrawal of an application or deletion; withdrawal ends consideration but may not erase a submitted record subject to a lawful duty. You may withdraw a separate consumer-health-data consent for future processing or sharing by contacting us. Withdrawal does not reverse processing already lawfully completed. We will assess a request under the law governing the particular information and explain any denial, permitted exception and available appeal or complaint route. We will not retaliate against you for exercising a right.

We may reasonably verify identity or a representative's authority before releasing protected information. Where an account request channel is available, you may use it without putting health answers in email.

Additional rights and response times depend on the record and law. The clinical Notice of Privacy Practices describes HIPAA rights. Our Consumer Health Data Privacy Notice explains additional handling and requests for consumer health data where that law applies. We do not assume that one law excludes all other protections.

Cookies, signals and other sites

The public film uses browser session storage for sound preference. The application needs session and security storage to keep an account signed in and protect requests. You can clear browser storage or sign out, although blocking required storage may prevent account features. We do not use this website to track your activity across other companies' sites for advertising. That practice does not change based on a Do Not Track or Global Privacy Control signal. Linked independent sites have their own practices.

Changes and contact

We post updates with an effective date and provide additional notice or ask for permission where required. Continued browsing is not a substitute for legally required consent. A new version does not silently change a consent already recorded.
